When you schedule an appointment with us, that time is reserved exclusively for you. We understand that emergencies happen, but to ensure we can serve all patients:

• We require at least 24 hours' notice for cancellations or rescheduling.
• Late Cancellations/No-Shows: Appointments cancelled with less than 24 hours' notice may be subject to a cancellation fee of $25.
• Repeated missed appointments may result in the inability to schedule future visits.

We are happy to bill your dental insurance as a courtesy. However, insurance policies are a contract between you and your insurance provider. You are ultimately responsible for all fees associated with your treatment, regardless of insurance coverage.

Please bring your current insurance card to every visit. Co-payments and deductibles are due at the time of service.

You agree to use our website only for lawful purposes. You are prohibited from:
